The reprocessing of flexible endoscopes places high personnel and organizational demands on endoscopy departments and reprocessing units for medical devices. Time pressure, complex processes, extensive training and ergonomic stress make it difficult for specialist staff to consistently comply with all hygiene guidelines.
As the Robert Koch Institute states in its official communication “Annex 8: Hygiene requirements for the reprocessing of thermolabile endoscopes”, most endoscopy-related infections are still attributable to non-compliance with existing guidelines and therefore also to the human factor.
Automated cleaning instead of manual brush cleaning significantly minimizes these risks and sources of error. It ensures reproducible results, reduces staff workload and at the same time increases process reliability for those responsible. For this reason, we have developed Endoprime, a standardized cleaning process that ensures greater hygiene, efficiency and safety in everyday reprocessing.
The problem
The reprocessing of flexible endoscopes poses major challenges for specialist staff on a daily basis. Manual brush cleaning is labor-intensive, time-consuming and difficult to validate.
The narrow working channels as well as connections, junctions and material transitions are often almost impossible to reach with brushes.
However, only residue-free reprocessing enables safe automated cleaning, disinfection and, if necessary, sterilization. Careless work can lead to residues or damage to the inner channels and increase the risk of cross-contamination.
The solution
The use of Comprex® reliably removes deposits such as dried blood, tissue residues and biofilms from the endoscope channels. The impulse flushing process using water and air works much more efficiently and hygienically than conventional manual brush cleaning – without any consumables or process chemicals. Even artificially introduced soiling such as lubricants and contrast agents, blood stains or color markers are safely removed.
The system can be seamlessly integrated into existing workflows and infrastructures. Instead of intensive brushing, only the endoscope connections need to be connected to the Endoprime system via a distributor.
The result
With Endoprime, we offer an automated, validatable and reproducible solution for the safe reprocessing of flexible endoscopes. The entire cleaning process is recorded digitally, enabling seamless documentation. Both the endoscope and the personnel are registered via barcode or RFID. At the same time, the system automatically and individually adjusts the cleaning performance to the channel sizes, lengths and branches of the endoscopes.
By cleaning all channels with fresh water, the separate cleaning discharge and the integrated leak test, demonstrably better cleaning results are achieved. The manifold and the endoscope can then be reprocessed as usual in the automated cleaning and disinfection process.

Automated endoprime cleaning achieves a log reduction of up to 4 levels and leads to a total log reduction of up to 13 levels after automated cleaning and disinfection in accordance with (DIN EN ISO 15883).
This achieves a reproducible and higher standard of hygiene than the previous manual brush cleaning method.
The cleaning performance of Endoprime was tested in accordance with the EN ISO 15883-4 / EN ISO 15883-5 standard without chemicals or cleaning agents.
The test results confirm the outstanding cleaning performance of Endoprime and show
that the same cleaning standards are met as for machine cleaning (pre-disinfection):
a) Removal of Geobacillus stearothermophilus (ATCC 7953) spores according to EN ISO 15883-4:2019, B.3.4
b) Evaluation of the cleaning efficiency according to EN ISO 15883-4:2019, 6.11 by protein and total organic carbon (TOC)

The appliance is 25 cm wide, 35 cm high and 20 cm deep and can be installed with a weight of 4.5 kg upright or suspended next to existing cleaning basins.
All you need is a power connection, a water connection with 2 bar and a compressed air connection with 4 bar.
The Comprex® impulse pressure of approx. 3 bar is safe for the endoscope channels during cleaning and is effectively below the pressure of commercially available mechanical flushing processes.
Comprex® can be used to effectively remove deposits such as dried blood, tissue residues, biofilms and artificially added soiling such as lubricants and contrast agents from the channels of flexible endoscopes. This ensures safe automated cleaning and disinfection in the washer-disinfector. The pressure load on the endoscope channels during the Comprex® process corresponds to the usual flushing processes in the washer-disinfector, so that the system can be used safely and gently.
Thanks to the automated cleaning of all channels with individually adapted cleaning parameters, all commercially available endoscopes – regardless of type or manufacturer – can be cleaned reliably and reproducibly. Specialized cleaning programs remove even stubborn residues in complex structures, such as in connections, branches or hard-to-access shadow gaps, without leaving any residue and in a validatable manner. The entire cleaning process, including the responsible specialist, the endoscope and all cleaning parameters, is then digitally documented.
